ENROLLMENT PROCEDURES
Step 1
Read & understand the Terms & Conditions, Benefits & limitations
Membership Fees,
Pre-existing Condition, etc.
Or you may request for a quotation by clicking
here, & fill out the "Inquiry form".
You will receive a quotation with all necessary info thru email.
You may contact us for questions or clarifications

Step 2
If everything is clear & understood & you are ready to enroll,
send an email to
sales@icare.net.ph, Attention: Raul, & ask for
the application forms. You may please specify how many you need. 
Application form,with instructions on how to pay,
will be sent to you thru email.

Step 3
Print the forms on legal size (81/2 x 13) bond paper back to back.
There should be two sheets only, first sheet is page 1 & 2 back to back,
and second sheet page 3 & 4 is entitled "Individual & Family Plan" back to back
Fill out the application forms. Be sure to answer the medical
questionnaires on Page 2.  Sign on all appropriate spaces
including the page entitled "Individual & Family Plan".


Step 4
Payment options

Option 1 - thru Union Bank of the Philippines
Pay the corresponding premium thru any branch of the Union Bank
of the Philippines.  USE DEPOSIT SLIP.  DO NOT USE "Bills Payment slip"
as you are a new member.  The account number & bank details
will be provided to you thru email with appplication form.
After payment, submit personally, or send thru mail or courier
the ORIGINAL APPLICATION FORM (2 sheets, 4 pages)
with a copy of the deposit slip to:

Option 2 - Pay direct to Head Office
Bring the properly filled out Application Form to the Head Office,
pay direct to Cashier & submit Application Form.

YOU MAY MAIL OR SEND THRU COURIERS THE ORIGINAL FORMS.
WE WILL ALSO SEND YOUR HEALTH CARD
TO THE ADDRESS YOU PROVIDED THRU 24 HR COURIERS.
YOU MAY ALSO AUTHORIZE SOMEBODY TO
SUBMIT FORMS OR PICK UP YOUR CARDS.
